Compensation: $61,000 - $72,000/year
Location: Brooklyn, NY
Schedule: 9 AM to 5 PM, Monday to Friday
Responsibilities as the Social Services Coordinator (LMSW):

In this position, you will collaborate with an interdisciplinary team to assess participant needs, coordinate services, and provide short-term interventions to support the well-being and successful transition of individuals in your care.

Responsibilities as the Social Services Coordinator (LMSW):
- Assessment & Intake: Perform intake activities, conduct psycho-social evaluations, and initiate discharge planning.
- Service Coordination: Lead treatment team meetings, develop treatment plans, and coordinate with external providers.
- Crisis Management: Provide crisis intervention and short-term therapy as needed.
- Family Engagement: Facilitate family meetings and promote permanent connections through various placements.
- Documentation & Compliance: Maintain required records and ensure compliance with regulations.
- Sanctuary Model Integration: Implement and model the Sanctuary Model to foster a trauma-sensitive environment.
- Licensing: Licensed Masters Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW).
- Education: Masters in Social Work required.
- Experience: 2+ years in child welfare or a related field, with clinical work involving youth and families.
- Skills: Strong communication, organizational, and crisis intervention skills.
- Flexibility: Ability to work a flexible schedule, including some evenings and weekends.
- Language: Bilingual in Spanish preferred!
