Social Services Specialist - Ascension

We Are Hiring:

• Department:  Crisis Services
• Schedule:  Part-time (20 hrs per week) Weekends; Sat & Sun 8:00 a.m. - 8:30 p.m.,  plus 3 - 4 on calls a month
• Center:  Ascension Presence Health Care
• Location:  Melrose Park IL  (On-Call location is Maywood, IL)

 

What You Will Do:

Assist clients by assessing their social services needs, solving problems, setting goals and obtaining required services.

• Conduct initial intake procedures.
• Assess needs and eligibility of potential clients.
• Participate in the initial assessment process, service plan of care development, and ongoing monitoring of each client's care.
• Obtain extensive social history from client and/or family member upon admission to program.
• Resolve client problems by clarifying the issue, determining the cause; selecting and explaining the best solution; expediting correction or adjustment; and following up to ensure resolution.

What You Will Need:

Licensure / Certification / Registration:

• Social Worker credentialed from the Illinois Department of Financial and Professional Regulation preferred.

Education:

• High School diploma equivalency with 2 years of cumulative experience OR Associate's
  degree/Bachelor's degree OR 4 years of applicable cumulative job specific experience required.

Additional Preferences:

Education: Bachelor's degree Social Services, Psychology, Human Services, Communications, or related is highly preferred

 

Transportation:  A personal vehicle is needed to conduct field work for this role.

 

Note:  To meet Medicaid Regulatory Guidelines associates must be at least 21 years old.
