Compensation: $70,000 - $75,000/year

Schedule: Monday to Friday

Our healthcare client is seeking a dedicated Social Work, Program Manager (MSW) in Staten Island, NY, to ensure seamless care and supervision of case managers working with older adults, focusing on identifying client needs, developing care plans, and overseeing service implementation.

Responsibilities as the Social Work, Program Manager (MSW):
- Client Assessment & Care Planning: Perform pre-screenings, initial intakes, and full assessments to identify client needs and preferences.
- Caseload Management: Carry a small caseload and provide clinical support for high-risk/high-needs clients.
- Supervision & Support: Supervise Master's and Bachelor's level case managers and social work interns, ensuring quality assurance and documentation oversight.
- Program Oversight: Authorize EISEP homecare and services, assist with urgent home visits, and manage PEC clients awaiting assignment.
- Community Engagement: Maintain relationships with community resources and participate in staff meetings, supervisory meetings, and NYC aging training.
- Data Management: Maintain statistical and programmatic data, ensuring familiarity with relevant DFTA standards and updates.
- Education: Masters Degree in Social Work or a related field.
- Experience: 3+ years of experience in a social service organization working with older adults (social work supervisory experience required).
- Technical Skills: Fluency in Microsoft Word, Excel, and Outlook (knowledge of DFTA database STARS is a plus).
- Certifications: SIFI certified or qualified to obtain certification.
- Attributes: Ability to work independently, handle multiple priorities, and create a collaborative work environment.
- Language Proficiency: Bilingual skills are preferred!
