Summary
The position provides high quality patient care. Demonstrates good patient and family assessment skills. Plans and implements treatments of individuals and families. Facilitates treatment psycho-diagnosis, treatment planning, group treatment, individual treatment, family treatment, and discharge planning. Functions well as a member of the multidisciplinary team and participates in community relations for the Senior Care Unit. Ensure that there is a community knowledge of the Senior Care Unit at Mille Lacs Health System. Participates in contact with all current and new referral sources.

Community Relations
- Makes contacts with referral sources such as nursing homes, assisted living, physician offices and other organizations serving the elderly, approximately 1 per month.*
- Participates in attendance at networking groups regularly to ensure MLHS is an integral part of the community services.*
- Follows up on calls to the unit inquiring regarding services.*
- In conjunction with select team members, makes individual appointments with referents to ensure information is up to date, questions are answered, etc.*
- Visits individuals in areas of LTC, Acute Care, Social Work, county offices.*
- In conjunction with select team members, maintains up to date referral source files, seeks new referral sources.*
Communications/Documentation
- Works individually and as a member of a team.*
- Demonstrates excellent verbal and written communication, facilitation, and negotiations skills.*
- Communicates with physicians in person, phone, Telemedicine techniques.*
- Prioritizes assignments and meets deadlines.*
- Completes intakes on potential patients, may coordinate admission process as needed.
- Along with director, researches and develops referral communication.
- Sends referral satisfaction surveys, tallies in conjunction with Quality Manager.*
- Designs written documents describing program services, including but not limited to: unit website, newsletter, admission information, workshop notices, unit brochure, newspaper articles.*
- Communicates with Program Director, including scheduled marketing/community relations meetings.*
- In conjunction with select team members, interacts with hospital marketing department in order to promote unit within hospital marketing activities, for direction and approval of media products.*
Educator
- Presents in-services as scheduled/requested to professional and community organizations on topics related to aging mental illness.
- Provides unit tours, participates in health fairs and other community events as approved.*
- Assists Program Director with advertising, marketing and CEU's available.
Discharge Planning
- Engages in timely, efficient discharge planning.*
- Works in close conjunction with patient, family, nursing staff and attending physician.*
- Shows knowledge of referral/placement process and available community resources.*
- Conducts 10 day - 2 week follow-up via phone.
Psychotherapy Sessions
- Conducts individual psychotherapy sessions as applicable.
- Communicates with family members for the purpose of continuity of care.*
- Conducts group therapy sessions. Displays theoretical knowledge of subject matter and effective communication skills.*
Social Evaluations
- Completes social evaluations on assigned patients as per policy.*
- Interviews, diagnostic and report writing skills.
Progress Notes
- Completes comprehensive progress notes in a timely manner.*
- Ensures notes are completed within 24 hours.*
Other Responsibilities
- Functions cooperatively as a member of the treatment team.*
- Communicates effectively with families and agencies, conveying a positive professional image.*
- Brings to Program Director's attention significant issues related to program functioning.*
- Utilizes feedback regarding performance and use supervision to improve performance.
- Responds to behavioral emergencies utilizing Security Alert techniques.*
- Assists with Milieu Management.*
- Acts as back up for ADLS.
- Attends staff meetings, trainings, and other events as negotiated with Director.
- Coordinates periodic social work communication/networking/quality sessions with other MLHS staff engaged in discharge planning and social work duties.*
Collaborate and assist other departments as needed.
Required Education and Experience
- Bachelor's degree in Social Service field required.
- Clinical skills, ability to work productively and effectively, ability to function well as a member of a multidisciplinary team, and ability to represent the program well in interaction both inside and outside the host hospital.
- Knowledge of age specific need including, but not limited to: life stage needs (grief, losses, end of life, etc.), severity of illness/intensity of service criteria, typical concurrent medical issues and continuity of care needs.
- Must be CPR certified and trained in de-escalation techniques, or able to obtain certifications within 90 days of hire.
- Must have experience in public speaking to small and large diverse groups.
- Must have knowledge of EMR.
- Computer skills - Outlook, Excel, PowerPoint, etc.
Preferred Education and Experience
- Master's degree as a Social Worker preferred, with 5 years mental health experience, one field placement experience or at least one year experience working with Adult Psychiatric patients, and master's level social worker supervision.

Position Type/Expected Hours of Work
Exempt: 40 hours per week, primarily Monday through Friday 8am to 4:30pm.
