Social Worker: Discharge Planning - Central Park Rehabilitation and Nursing Center

Central Park Rehabilitation & Nursing Center, located in downtown Syracuse just a short distance from Syracuse University and local Hospitals, is looking to add a dynamic and experienced Social Services leader to our team. 

Responsibilities of this position will include:

Leading the discharge planning process for the facility on a 40 bed Short Term Rehabilitation Unit.

Providing support to residents and their families throughout their admission, stay and discharge from the facility.

Develop, maintain and utilize a current list of community services for staff, residents and their families.

Facilitate all aspects for a safe discharge plan which includes skilled home care, therapies and DME procurement as required.

Participate as a member of the Interdisciplinary Team while addressing resident's psychosocial, mental, or emotional needs.

Demonstrate a positive work attitude and excellent customer service skills.

Maintain compliance with state and federal regulations.

Assist with managing the Social Services Department in coordination with the Director as needed.

Perform other work-related duties as assigned. 

Qualifications:

Bachelor's degree in Social Work or a related field ideally with a minimum of 2 years Skilled Nursing Facility experience, with emphasis on discharge planning but not necessary.

Excellent communication skills

Ability to work in a fast-paced environment.
