Here is what is needed to be part of this amazing team:

·         Bachelor’s degree required. -Master’s degree strongly preferred.

·         Minimum three (2) years’ experience as a social worker in a healthcare related setting.

·         Long term care and/or rehab experience preferred 

·         Knowledge of the rules, regulations, and guidelines that govern nursing care facilities.

·         Excellent Communication skills – ability to communicate with personnel, residents, family members, visitors, government agencies/personnel and the general public.

·         COVID vaccination is required.

Responsibilities: 

·         Pre- Admission & Discharge Planning

·         Psycho-social assessments of residents

·         Work with interdisciplinary team members to create & implement care plan for residents

·         Provide support & resources for residents and residents families to achieve optimal care of residents

·         Maintain resident confidentiality.

·         Advocate for residents and act as a liaison between residents and resources in the community

·         Ensure residents social & emotional needs are met

·         Promote resident independence & positive self image

Additional Differentials including-

Pay in Lieu of Benefits: extra $1.50

Waiving medical only: extra $1.50 per hour

Short Pay: $2.50 per hour
