Social Worker - Center for Nursing and Rehabilitation

As a Social Worker, you will collaborate with the Director to plan, organize, and oversee the Social Services Department's day-to-day operations, ensuring the highest level of care and support for our residents.

Salary: $35/hr

Key Responsibilities:

• Conduct interviews with residents and families to gather social histories.
• Engage residents and families in developing social service programs whenever possible.
• Provide practical solutions for social and environmental challenges, including securing financial assistance, discharge planning, and referring to community agencies for specialized support.
• Work closely with administration, medical and nursing staff, and other departments to coordinate social service programs.
• Participate in state and federal inspections and surveys as needed.
• Communicate residents' social, emotional, and psychological needs to the care team, including medical staff and physicians.

Requirements:

• Minimum of 2-year experience in a Skilled Nursing Facility (SNF).
• Preferably a degree in Social Work or a related field.
