Job Title: Senior Social Worker

Responsibilities:

• Conduct outreach to Blue Shield of California Promise Health Plan members.
• Assess social drivers of health needs through comprehensive evaluations.
• Provide intensive interventions and support to address identified needs.
• Collaborate with healthcare teams to develop and implement care plans.
• Maintain accurate documentation and case records.

Qualifications:

• Master's degree in Social Work (MSW) required.
• California LCSW license preferred.
• Minimum 3 years of experience in a clinical or community setting.
• Strong communication and interpersonal skills.
• Knowledge of social determinants of health.
