Position Summary:
The Speech Pathologist diagnoses, plans, and directs communicative and swallowing efficiency. Determines need for referral to specialists in medicine, dentistry, education, psychology, sensory and related areas. Acts as a consultant to medical and other professional personnel in the basis of speech pathology.
Duties/Responsibilities:
• Provides ongoing interdisciplinary assessment of the patient; evaluates and diagnoses patients referred for services using independent, professional judgement to determine the appropriate plan of care based upon the patient’s disability, age, and psychosocial needs.
• Develops and evaluates the plan of care in partnership with the patient, representative (if any), and caregiver(s). Treats patients in accordance with professional standards of care and practice based upon the patient’s disability, age, and psychosocial needs.
• Provides patient/family/caregiver education based upon the patient’s disability, age, and psychosocial needs as well as counseling as needed.
• Prepares clinical and professional documentation in a timely manner and communicates plan of care to other professionals.
• Provides services that are ordered by the physician as indicated in the plan of care.
• Exhibits professional communication and management skills.
• Communicates with all physicians involved in the plan of care and other health care practitioners (as appropriate) related to the current plan of care.
• Participates in the HHA’s QAPI program and HHA-sponsored in-service training.
Education:
Master’s Degree in Speech Pathology.
Experience:
One or more years of clinical experience preferred.
Licensure/Certification:
Certification by American Speech-Language-Hearing Association, Licensure in Speech-Language Pathology by the Kentucky State Board of Examiners for Speech-Language Pathology and Audiology.
