A Substance Abuse Rehabilitation Program located in Queens is seeking Full-Time & Part-Time Substance Abuse Counselors (CASAC, CASAC-2, CASAC Advanced/Master, LMSW, LMHC
Type: Perm
Schedule: Part-Time and Full-Time hours available (many openings)
Location: Fresh Meadows, NY
Salary Range: $55,000 - $62,000
Responsibilities:
• Under the direction of the Director, performs psycho-social assessments, assists in the formulation of recovery care plans, maintain compliant electronic records, and conducts educational and group meetings with patients. Makes appropriate referrals when indicated.
• Performs admission interviews and assessments in terms of social background and motivation for involvement in continued care.
• Assess Mental Status and make appropriate referral when applicable.
• Collaborate with various criminal justice agencies.
• Obtains information from the patient relative to his/her chemical dependency.
• Provides patients with education that will resolve or minimize identified problems during the period of chemical detoxification or rehabilitation as well as to promote and maintain abstinence after discharge.
• Collaborates with other disciplines in the formulation of a multi-disciplinary approach to patient care.
• Assists with admission, discharges and transfers, if applicable.
• Observe, monitor and administer patients giving urine samples when assigned.
• Participates in the development and implementation of the Quality Improvement program.
• Documents all relevant information in patient electronic medical records within prescribed time frame.
• Prepares diagnostic summaries, recovery care plans, maintains progress notes, and contacts as required, with EAP/Referrer and health insurance provider.
• Develops patient continuing care plan and document patient discharge summaries in chart.
• Refers patients to specific agencies for continued care as needed.
• Participates in the formal lecture/discussion educational programs of the department by giving and/or moderating groups.
• Keeps Director and/or Administrators informed regarding changing conditions of patient and/or other pertinent observations.

Requirements:
• Must be Licensed CASAC, CASAC-2, LMSW or LMHC
-Must have at minimum 1 year of inpatient substance abuse experience
• One-year recent clinical experience including demonstrated efficiency in patient assessment
• Experience in group and individual counseling
• Prior experience with Electronic Medical Records (ECR) preferred
• Must be CPR certified, if transporting patients must be certified in First Aide
