Summary: Under the Manager of Discharge Planning the Discharge Planning Supervisor functions in a supervisory administrative and/or program management capacity within the Department of Discharge Planning. Collaborates with affiliate leadership and other discipline leadership to meet objectives.
Responsibilities: In collaboration with the Manager of Discharge Planning supports and implements customer-focused coordinated delivery of quality assurance/quality improvement services related to discharge planning services. Participates in the evaluation of services and processes to identify opportunities for improvement quality enhancement and increased effectiveness and efficiencies. Assists the Manager of Discharge Planning with systems to assess patient family and community needs for services and programs. Maintains relationships with post-acute providers Performs all duties of the Discharge Planning staff (Transition Care Coordinators and Prior Authorization Specialists) and demonstrates expertise in the specialty. Provides supervision and work direction to Discharge Planning staff. With the Manager of Discharge Planning evaluates employee work performance through review and completes annual performance evaluations as required. Participates in counseling of supervisees around performance issues as required. Coordinates work assignment of employees and ensures adequate coverage for discharge planning area of responsibility. Participates in system-wide coverage for department and/or scheduling issues. Supports all through-put and capacity management activities for the Brown University Health System. Provides guidance to staff in resolution of more complex issues related to discharge planning. Participates in departmental studies grants Operational Excellence Projects committees and quality assurance activities. Maintains records prepares reports and composes correspondence relative to supervisory administrative and/or program management functions as necessary. Performs other duties as assigned.
Communicates policy and procedures to area personnel.
Provides input on hiring. Provides input to policy & procedure development.
Maintains and enhances professional expertise through educational activities.
Adheres to Brown University Health Hospital and departmental policies and procedures to maintain a safe environment of care and compliance with all laws regulations and standards.
Other information: Minimum of 4 years of experience preferably in a health care setting with an emphasis on populations to be served in the assigned area. Experience in supervision program development or administration required. Demonstrated knowledge and skills necessary to provide patient care and to develop or administer discharge planning services. Initiative and ability to work independently (self-direction) while thriving in a setting requiring collaboration and teamwork for maximal efficiency and effectiveness. Attention to detail that ensures follow through on all initiatives implemented and all projects undertaken Working knowledge of quality and safety improvement strategies and methods Customer service orientation
Qualifications - Education:
Bachelor�s Degree in Nursing from an accredited National League of Nursing (NLN) program with current license to practice as a Registered Nurse in the State of Rhode Island preferred.
Qualifications - Experience:
Analytical skills necessary to evaluate patients programs and procedures.
Administrative and supervisory skills necessary to address discharge planning staff functions in the department and/or assigned clinical area.
Demonstrated ability to exercise independent judgment. Refers specific complex problems to Manager of Discharge Planning or designee where clarification of departmental policies and procedures may be required.
Strong leadership skills and excellent interpersonal and facilitation skills. Ability to effectively respond to time sensitive issues and meet deadlines.
WORK ENVIRONMENT AND PHYSICAL REQUIREMENTS:
Works in a normal office environment with exposure to patient care.
SUPERVISORY RESPONSIBILITY:
Will supervise discharge planning staff as appropriate for service area.
