Job DetailsSocial Worker/Discharge Planner - 40 hrs DaysMust have DC License in handShifts: 8am – 4:30pmDay and rotating weekendsSTART DATE: 10/14Will accept RN Discharge Planner or Licensed Social WorkerSocial worker must be eligible for LMSW/LGSW/LCSW - they must have taken and passed the exam alreadyWork Setting skills Required: Cerner, Acute Hospital, Insurance Company, Long Term Acute Care/Rehab/Skilled NursingWork Setting skills Preferred: Home Health, Hospice, telephonic, previous charge experienceCase Management/Utilization Review skills required: Admission Criteria, Benefits Eligibility, Care coordination, Discharge Planning, Needs Assessment/ Order DME, Plan of Care, Pre-Cert Review, Prior Authorizations, Worker’s comp case managementRegulatory Skills Required: CMS: Centers for Medicare and Medicaid Services, Department of Health, HIPAA (The Health Insurance Portability and Accountability Act), The Joint Commission/ Core Measure/National Safety GoalsRegulatory Skills Preferred: Worker’s CompensationAny Special Requirements: Must have experience with discharge planning, obtaining facility choices, sending referrals, issuing IMM and other Medicare lettersPatient Diagnosis: Geriatrics, ICU, Heart and Vascular, General Surgery, General Medicine, MICUHolidays – as neededWeekends – Every 5-6 weekends must work one shift)Cerner is requiredUniform: Business Casual-Associate’s appearance should be clean, neat and professional
