**Job Description:**
- Provide therapy to patients requiring physical rehabilitation.
- Work within a multidisciplinary team to create comprehensive care plans.
- Maintain patient records and continuously monitor progress.
- Utilize EPIC EMR for documentation and patient data management.
- Attend to shift duties from 07:00 to 15:30, Monday through Friday.
- Adhere to the dress code with scrubs in the designated color.
- Fulfill any floating requirements within the facility based on need.
- Ensure timely and compassionate care to all assigned patients.
- Collaborate with healthcare team members to enhance patient outcomes.
