"Job Summary:
Medadventures is seeking a dedicated and compassionate Physical Therapist to join our team at a skilled nursing facility. In this role, you will work with an interdisciplinary team to help residents improve their mobility, strength, and overall quality of life. You will have the opportunity to make a real impact by developing and implementing individualized care plans for residents recovering from illness, injury, or surgery.Responsibilities:

• Perform thorough evaluations of residents to assess their physical conditions and mobility challenges.
• Develop personalized treatment plans to restore function, reduce pain, and prevent disability.
• Utilize a variety of therapeutic exercises, manual therapy, and modalities to meet each patient’s goals.
• Collaborate with nurses, physicians, and other healthcare staff to ensure comprehensive patient care.
• Educate patients and families on post-treatment exercises and strategies to maintain progress.
• Accurately document patient progress and maintain records in compliance with regulatory standards.

Qualifications:

• Doctor of Physical Therapy (DPT) or equivalent.
• Active licensure as a Physical Therapist.
• Experience in a skilled nursing or long-term care setting preferred.
• Strong interpersonal and communication skills to work effectively with patients, families, and healthcare teams.
