- Provide rehabilitation services to residents
- Collaborate with a team to develop and implement care plans
- Document patient progress and update records as needed
- Communicate effectively with residents, families, and staff
- Ensure compliance with all applicable regulations and standards
- Occasionally travel to Valley View and Madison Manor facilities
- Participate in team meetings and training sessions as required
- Maintain a clean and safe work environment at all times
