**Job Description:**
- Assess, plan, organize, and participate in rehabilitative programs to improve mobility, relieve pain, increase strength, and decrease or prevent deformities.
- Implement and adjust the physical therapy treatment plan with patient/family input
- Provide ongoing patient/family education and training and offer discharge instructions with referrals as appropriate.
- Act as a resource for staff, families, and physicians about rehabilitation services and equipment.
- Collaborate with physicians to establish and evaluate treatment programs, plan of care, goals, and discharge plans.
- Document evaluations, treatment sessions, and any educational or discharge-related activities according to guidelines.
- Maintain ethical standards, confidentiality, and punctuality.
- Engage effectively in verbal and written communication with team members.
- Participate in staff meetings and professional development activities.
- Use materials and equipment safely and cost-effectively
- Foster a positive environment for coworkers and patients and encourage teamwork.
- Participate in professional development, and complete mandatory education and competencies.
