**Experience:** New graduates are welcome, Skilled Nursing Facility (SNF) experience preferred

**License:** State license required

**Certifications:** COVID test required 72 hours prior to start date, with results issued 48 hours before starting

**Job Description:**
- Ensure prescreen is filled out and attached upon candidate submission
- Complete county criminal check for the past 7 years for all counties lived and worked
- Complete federal criminal check for 7 years or more
- Conduct multi-state criminal search for 7 years or more
- Perform nationwide sex offender search for 7 years or more
- Conduct HHS/OIG search
- Perform GSA search
- Conduct social security number trace
- Provide 2 Step PPD or Quantiferon test results, or proof of positive PPD with a chest x-ray
- Conduct SAM search
- Complete Oregon state background check (facility run)
