Major Responsibilties
- As the SNF-ist, you will act as a Skilled Nursing Facility’s leading provider of internal medicine services on Saturday and Sunday.
- You will collaborate with the facility’s interdisciplinary team and physicians to reduce hospitilizations on weekends.
- You will work with the entire facility’s patient population - short and long-term residents with the assistance of the facility’s nursing staff and guidance from the facility’s medical directors and attending physicians
- Evaluation of new-admission visits
- Coordination of discharge visits
- Evaluations for patients exhibiting changes in condition
- As a SNF-ist, you will provide types of care, including but not limited to:
- Advanced Care Planning
- Provide comprehensive health assessments, differential diagnosis, and prescribe treatments under the protocol in the direct management of acute and chronic illness and disease
- Prescribe all necessary medications and treatments
- Perform comprehensive physical examinations of patients
- Develop and implement patient management policies and procedures
- Facilitate referrals to other healthcare professionals and medical facilities
- Communicate with attending physicians and/or specialists regarding patient care

Qualifications
- Advanced Practice Licensure (APN) with a Florida license
- Must have an active license and DEA number.
- Previous experience in nursing or other medical fields.
- Ability to build rapport with patients and their loved ones.
- Ability to thrive in a fast-paced environment.
- Excellent written and verbal communication skills.
- Strong leadership experience with emphasis on teamwork in a healthcare setting.
- Master’s degree required
