Job status: Full Time (8:00-5:00pm) This position may require weekend or evening to address/support clients
JOB SUMMARY:
The WFS Bridging Case Manager will primarily work with eligible individuals in the Lewiston/Auburn area. The Case Manager works with families and individuals to develop an individualized service plan that includes appropriate services and supports that meet the needs of the family/individual. The Case Manager informs the family of services and supports and assists, when needed, in coordinating services. This position requires a valid driver license, reliable transportation and personal vehicle auto insurance with at least $100,000/ $300,000 liability limits to transport clients.
Essential Duties and Responsibilities
The essential functions include, but are not limited to the following:
- Screen and enroll eligible participants in the whole family services program and provide linguistically and culturally appropriate service coordination.
- Assist the program director with the design, translation, and production of outreach flyers and distribute the outreach flyers to the community.
- Conduct a comprehensive needs assessment with participants
- Develop a holistic individualized family plan for each whole family services program participant.
- Provide wrap-around linguistically and culturally appropriate bridging case management for participants
- Assist with the transportation and support of eligible WFS clients to mandatory court appearances as necessary - use of personal vehicle required.
- Proactively inform clients of MEIRS’s available programs and services as well as services available in the area.
- Continually seeking ways to optimize the efficiency and effectiveness of bridging case management services.
- Equitably and consistently adhere to legal, organizational, and program-specific laws and policies.
- Ensure that all formal client interactions are retained thoroughly, accurately, and expeditiously in the client management database.
- Collaborate with clients to develop, implement, and regularly monitor goals and strategies to achieve them
- Establish and maintain respectful, professional, culturally-sensitive, and appropriate boundaries with clients.
- Provide or facilitate education and assistance focusing on the client’s needs, rights, and responsibilities
- Assist clients and their families in applying to external resources
- Conduct home visits to client homes as necessary
- Cultivate and maintain strong relationships with external shareholders related to bridging case management.
- Regularly communicate reports, data, and other salient information to the whole family service director
- Promote organization programs and services via in-person community engagement and social media
- Accurately and thoroughly document expenses and adhere to a program-specific budget.
- Other duties assigned by the supervisor.
Required Knowledge, skills and abilities
- Familiarity with the new Mainer’s experience in the United States is required; first-hand experience is highly valued
- Near-fluent English language speaking, understanding, reading, writing skills
- Ability to speak, understand, read and write one or more of the following languages is preferred: Portuguese, Maay Maay, Somali, Lingala, Kiswahili, French
- Direct experience delivering direct human service
- At least a high school level of education; college experience strongly preferred.

- Arrange for interpreter services, if necessary.
- Prepare narrative and statistical reports and submit to supervisor
- Enter data into the MEIRS systems/other databases MEIRS uses in recording all activities in a timely and accurate fashion.
- Complete and maintain physical file documentation in case files in a timely and confidential manner.
- Attend and participate in staff meetings, supervisory meetings, other meetings, and training programs.
- Assist in the coordination of activities with other MIERS programs and work as a member of a team.
- Maintain a valid driver’s license and clean driving record.
- Submit timely: timesheets, mileage and personal expenses and other required administrative tasks
- This position requires a valid driver license, reliable transportation and personal vehicle auto insurance with at least $100,000/ $300,000 limits to transport clients.
Qualifications:
- One or more years of similar field experience. Minimum high school diploma is required.
- Associate Degree/Bachelor's or Master's degree in a related field (Social Work, Psychology, Counseling, etc.)is preferred.
- Previous experience in case management or a related field with a proven track record of leadership.
- Strong interpersonal and communication skills.
- Ability to work effectively with diverse populations and communities.
- Knowledge of relevant software and databases for case management.
- Strong organizational and problem-solving skills.
- Valid driver's license and reliable transportation (if required for client visits).
- Cultural competence and sensitivity.
- Flexibility and adaptability to meet the evolving needs of the program.

Requires work with external customers or the public, work with others in a group or team, working indoors in environmentally controlled conditions. Frequent meetings with clients and partners both outside and inside the office. May be requested to work outside of scheduled hours as defined by the needs of the resettlement program. Will attend occasional off-site meetings. This position requires working indoors in environmentally controlled conditions, with the least exposure to sounds and noise levels that may be distracting or uncomfortable. Job tasks are performed in close physical proximity to other people.
