Wound Care Coordinator RN LPN - Confidential

This is a management position with weekends on-call rotation required

Wound Care Nurse Duties Include but are not limited to:

• Responsible for accurate and timely documentation of wounds in the nurse's notes at least weekly
• Responsible for all treatments required by licensed nurses on your shift and ensuring the wound care treatments are carried out at other times.
• Evaluate high-risk patients for pressure reduction intervention methods
• Conduct body assessments and documentation on all new admissions per policy
• Round with doctors weekly to assist with assessing residents

Wound Care Nurse Requirements/Qualifications:

• Must be a self-starter, hard-working and dedicated professional willing to join our team.
• Current and valid RN or LPN license in the State of IL
• Active CPR Certificate
• Point Click a Plus
• Wound care certification desired
• Excellent communication skills
• Experience in LTC preferred but not required
• Positive attitude and a team player
