• Must have active state license
• BC or BE required
• Weekdays, Days, No call
• Looking for 3 - 4 days a month or ideally one day a week of procedure coverage
• Botox injection for chronic migraine, cervical epidural steroid injection, facet joint injections
• RFA's preferred, medical branch blocks, other B&B pain procedures would be great but not required
• Credentialing needed
• DEA needed
