Job Title: Social Services Coordinator

Responsibilities:

• Develop and implement social service programs for vulnerable populations.
• Conduct assessments to identify client needs and create individualized service plans.
• Collaborate with community organizations and stakeholders to enhance service delivery.
• Provide case management and support to clients, ensuring access to resources.
• Monitor and evaluate program effectiveness, making adjustments as needed.

Qualifications:

• Bachelor's degree in Social Work or related field.
• 2+ years of experience in social services or community outreach.
• Strong communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Knowledge of local resources and services.
