About the Job
- Provide supported employment and vocational case management services to Assertive Community Treatment (ACT) Team patients as part of the collaborative and integrated ACT Team.
- Provide direct and indirect services that actively facilitate competitive job acquisition and retention based on patient preference and abilities.
- Employ evidence based, supported employment practices in the delivery of services.
- Provide on-site job support and supervision as indicated.
- Conduct job development activities.
- Provide Functional Support Services as appropriate.
- Assist patients in maintaining community living by teaching them activities of daily living and personal care.
- Promote independent and productive habits of patients in the community.
Qualifications
Bachelor’s Degree in the Human Services area with emphasis in mental health and/or vocational rehabilitation and 1-2 years’ experience; other combinations of education and experience may be substituted.
Salary
This position pays $21/hour with a $2,000 sign on bonus
