SUMMARY
The ACT Coordinator directs overall program operations, supervises and evaluates the staff and provides direct services to consumers as a member of the teams. Works with ACT staff to ensure quality, community based services are provided to meet individualized needs and promote positive outcomes for persons served. Establishes mechanisms to monitor and ensure program compliance with licensure, DMAS and other regulatory standards. Provides guidance and leadership to team members.
ESSENTIAL FUNCTIONS/PRINCIPAL DUTIES AND RESPONSIBILITIES OF POSITION
- Hires, supervises, evaluates and schedules the work of ACT staff members in conjunction with Team Leads.
- Establishes methods and procedures for providing the best service delivery system utilizing available staff, budget, and other resources.
- Coordinates assessments to individuals referred to the program.
- Assures health and safety regulations are followed and sets periodic safety drills and inspections.
- Makes agency health and safety recommendations as part of the Health and Safety Committee.
- Coordinates services and serves as liaison with other agencies: hospitals, courts, social services, state offices and others in the communities.
- Prepares and maintains program records and statistical data and prepares and maintains related reports.
- Attends interdisciplinary, inter‑agency and community based meetings relating to individual treatment.
- Attends and supports daily staff meeting with team members
- Participates in budget preparation of program, and makes budget adjustment recommendations as appropriate. Assures established budget is implemented and monitored closely throughout the fiscal year.
- Recommends repairs and renovations to facility and facility equipment.
- Ensures compliance with CARF, Medicaid, DBHDS, and other regulatory standards through development and refinement of program policies and procedures and quality assurance of necessary individual documentation.
- Composes letters, memorandums and program reports that address program goals, individual issues, facility updates and recommendations and program progress.
- Reports and monitors ACT service provision. Ensures that ACT services align with the treatment plan and are provided in settings that meet the needs of the individual served, including home, shelter, hospital, work, and any other appropriate community setting.
- Manages case assignments and works with staff to develop and coordinate the initial assessment and treatment plan.
- Directs and coordinates the following: re-authorization process to ensure medical necessity of services; comprehensive evaluation of service needs as well as risk factor; targeted interventions that address assessed areas of need; and ensures services are reimbursable by payer sources.
- Provides direct services to ACT persons served as a member of the team. Meet with persons served to address concerns, improve services and resolve complaints when possible.
- Conduct staff recruitment, interviewing, hiring, work assignments, orientation and training for the program.
- Supervises medical records management assuring maintenance of the record in compliance with agency policies, Medicaid, and other third-party payment requirements.
- Conducts chart reviews and provides training to staff on regulatory requirements and best practice standards.
- Carries out and documents quality-assurance activities to ensure high quality service provision.
- Develops and maintains program policies and procedures.
- Performs on-call duty for crisis intervention and is available for staff supervision and consultation.
- Performs other related duties as assigned.

FORMAL TRAINING AND WORK EXPERIENCE EXEMPLIFYING KSAs
A minimum of Master's Degree in counseling or social work. Must be a licensed mental health professional (LMHP) as defined by Virginia Department of Health Professions. At least five years of clinical experience providing treatment to persons with severe and persistent mental illnesses and co-occurring disorders. ACT or other Assertive Outreach team experience is preferred. Two years of supervisory and program management experience required. The person in this position must be knowledgeable about the ACT Fidelity Model and applicable licensure and DMAS regulatory standards.
LICENSURE: Valid Virginia Driver's License and acceptable driving record as reported by the Department of Motor Vehicles are required for persons using a personal or agency vehicle in the performance of their duties. Licensed Mental Health Professional in good standing with the state of Virginia (e.g. LPC, LCSW) required.
PHYSICAL REQUIREMENTS: Must have the use of sensory skills in order to effectively communicate and interact with other employees and the public through the use of the telephone and personal contact as normally defined by the ability to see, read, talk, hear, handle or feel objects and controls. Work is performed in an office environment. The incumbent may be exposed to hostile and abusive individuals. Evening hours will be schedÂuled when there is other active individual service occurring at the clinic. Physical capability to effectively use and operate various items of office related equipment, such as, but not limited to, a personal computer, calculator, copier, and fax machine. Some walking, moving, carrying, climbing, bending, kneeling, crawling, reaching, and handling, sitting, standing, pushing, and pulling.
