Job Description
To provide professional social services to residents and family members, with emphasis on addressing the psycho social needs of residents and family members. Coordinating needed services with discharge planning. Directs and implements Activity Program which involves residents, family members and Hospital staff. The Activity Director provides patient care under the direction and supervision of the RN LPN. The Activity Director assists in the patients care, anticipating patient family needs in a proactive fashion. The Activity Director individualizes and modifies care according to the patients needs. The Activity Director collaborates with other health care team members to enhance the provision of patient care. The Activity Director will possess and utilize effective technical skills for the provision of safe and appropriate care, including pharmacology knowledge and medication administration as needed.
This position is weekends only for 8-10 hour shifts.
