Job Title: Addiction Counselor / Corrections Social Worker / Discharge Planner
Location: Marienville, PA
Pay: $27.00 hour
Hours: Monday to Friday, 8:00 AM - 4:00 PM (30-minute lunch break)
Position Overview:
We are seeking a dedicated and compassionate Human Services or Mental Health Professional to join our team as an Addiction Counselor. In this role, you will provide critical counseling and support to individuals with a history of substance abuse, particularly those transitioning from incarceration. You will help facilitate their journey to recovery by assisting with discharge planning, medication-assisted treatment (MAT) therapy, and connecting them with community resources prior to their release.
Key Responsibilities:
- Deliver social work, counseling, and case management services to enhance the social functioning of individuals and facilitate their adjustment in the community.
- Collaborate with reentrants to develop personalized treatment plans, ensuring they have access to necessary services, benefits, and resources.
- Schedule and coordinate community appointments for medication and treatment, fostering smooth transitions for individuals upon release.
- Maintain comprehensive records, tracking and updating databases for all individuals released from the facility.
- Conduct assessments, including initial evaluations, 6-month reviews, and discharge assessments, to monitor progress and identify ongoing needs.
- Communicate effectively with Center Staff, SCI Staff, and Parole Staff regarding treatment planning and client progress.
- Support individuals with positive drug screens by conducting assessments and referring them to appropriate treatment options.
Training Opportunities:
- Comprehensive on-site training will be provided during your scheduled shifts.
- Department of Corrections (DOC) training is required within the first six months of employment. This 3-week training will take place in Elizabethtown, PA, with travel expenses reimbursed and overnight accommodations provided during the week (Saturday and Sunday at home).
- You will be reimbursed for travel expenses.
Qualifications:
- Bachelor’s degree in Social Work, Human Services, or a related field is required.
- Proven knowledge and experience in case management, discharge planning, and community resource coordination.
- Familiarity with counseling techniques and interventions aimed at addressing the psychosocial, economic, and physical needs of individuals and families.
- Understanding of common physical and mental health issues and their social, economic, and psychological implications.
