**This role may sit in Memphis or Nashville, and will offer 4 days in office and 1 WFH day**
JOB SUMMARY
We are seeking a highly organized and detail-oriented Full-Time Administrative Follow-Up Nurse to join our Health Care Services team. This role focuses on both clinical and administrative tasks, ensuring that patients receive timely follow-up care while supporting the operational needs of the health center. The Administrative Follow-Up Nurse will coordinate patient care, manage communication between healthcare providers, and handle administrative duties related to patient follow-up and care continuity.

Patient Follow-Up: Proactively reach out to patients post health center visits or after procedures to assess their recovery, answer questions, and ensure adherence to care plans.

Care Coordination: Work closely with providers, specialists, and other health center staff to schedule follow-up appointments, coordinate additional care, and facilitate referrals.

Patient Education: Provide patients with comprehensive information about their treatment plans, medications, and post-care instructions.

Administrative Support: Manage and update patient records in the electronic health record (EHR) system - Epic, ensuring all follow-up communications and care plans are documented accurately.

Monitor Patient Progress: Track patient outcomes through phone calls, emails, or virtual check-ins to monitor progress, and identify any issues or complications that may require further medical attention.

Liaison: Serve as the communication point between patients and healthcare providers to address questions, concerns, or scheduling issues.

Documentation and Reporting: Ensure all follow-up activities are documented in compliance with PPFA, affiliate and health care services standard operating procedures, and prepare reports on patient follow-up outcomes for Chief Clinical Officer and Lead Clinician review.

Appointment Management: Assist with scheduling and rescheduling appointments, procedures, or follow-ups as needed, based on patient availability and provider schedules.

Patient Outreach: Conduct outreach campaigns for patients who have missed follow-up appointments or who need reminders about their care plan adherence.

Experience with patient care coordination and administrative processes preferred.

Must be an effective time manager, able to work without direct supervision, and demonstrate sound judgment and decision-making skills.

Knowledge of healthcare regulations, including HIPAA compliance.

Must be able to interface positively with patients and staff.

Must have integrity, energy, and enthusiasm.

Accepts a flexible work schedule including weekend and evening hours as necessary.

Must be able to drive with valid license and insurance and lift and carry 25lbs. unassisted.
JOB REQUIREMENTS:
EDUCATION: BSN preferred but not required.
EXPERIENCE: Minimum of 3 years of experience in clinical nursing, case management, or a related administrative role in a healthcare setting.
Other Requirement:

Registered Nurse (RN) with a valid state nursing license.
