JOB DESCRIPTION
Summary/Overview
Admissions Case Managers conduct initial home visits in the Greater Boston area to assess eligibility, evaluate needs and enroll elder's to the State Home Care Program as well as offer additional community resources. They will develop and implement a comprehensive service plan and screen for community resources. Admissions Case Managers work collaboratively with informal and formal supports, vendors and other community agencies.
Essential Functions

- Conduct home visits to assess, evaluate and enroll elders for state home care services and/or make recommendation for community assistance.
- Utilizing EOEA guidelines develop a care plan based on individual unmet needs of the elder. Ensure interdisciplinary review.
- Present all new cases at Admissions Review Team for approval and interdisciplinary guidance
- Implement the plan based on utilizing BSHC and other community resources
- Maintain consumer and care plan monitoring through ongoing vendor communication and phone calls to consumer and informal supports until case has been transferred to ongoing case manager.
- Create the consumer record and complete documentation of all significant communications from vendor, consumer and informal supports.
- Adhere to BSHC and EOEA compliance standards regarding timeliness of visits, documentation, and follow up.
- Maintain a working knowledge of BSHC and other community programs or have the skill to access this information.
- Develop and hone skills to advocate for consumers in a wide variety of arenas.
- Contribute to organizational life through active participation in team activities, committees, and work groups. Promote a positive working environment through cooperative and respectful interaction with others.
- Consult with Team Managers and other staff as needed
- Conduct ongoing outreach efforts in order to ensure access to services for all consumer groups.

Competencies
- Sensitivity to elder issues and commitment to maintaining clients at home with dignity.
- Ability to effectively communicate in person and by telephone.
- Must have computer experience, and be able to perform basic data entry and effectively utilize MS Office Suite and Boston Senior Home Care's database.
Work Environment
This job operates in a professional office environment. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines. The role also requires the employee to work out in the field, visiting consumers in their homes.
Physical Demands:
The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job:
- Incumbent must be able to carry a 15 to 20lb load to consumers' homes, must be able to travel to the community to perform visits at consumers' homes (may have limited accessibility), and travel under all weather conditions.
Position Type/Expected Hours of Work
This is a full-time 35 hour per week position (7 hours per day Monday through Friday).

Required & Preferred Education and Experience
- BS or BA degree required, and licensure preferred.
- Relevant work experience preferred and sensitivity to elder and diversity issues.
