Job Overview:
We are seeking a motivated and organized Admissions Coordinator to join our team. In this role, you’ll be primarily responsible for assisting in managing facility referrals and help facility overcome census barriers.
Responsibilities:
- Actively respond to referrals and inquiries, ensuring prompt and thorough follow-up.
- Provide clear and helpful information about our services, care plans, and facility amenities.
- Assist in addressing any questions or concerns related to the admissions process.
- Collaborate with clinical staff, social workers, and other departments to support a smooth admission experience.
- Coordinate communication with insurance companies, physicians, and external referral sources to facilitate admissions.
- Provide detailed information about services, care plans, and facility amenities.
Qualifications:
- High School Diploma or equivalent required; an Associate's or Bachelor's degree in Healthcare Administration, Social Work, or a related field is a plus.
- At least 2 years of experience in admissions coordination, preferably in a skilled nursing facility or healthcare setting.
- Strong organizational and time management skills, with a proactive approach to managing referrals.
- Excellent interpersonal and communication skills.
- Ability to handle sensitive and confidential information with discretion.
