What you’ll do
We will look to you to represent our life plan community and our organization to the greater community. You’ll work with case managers, social workers, and discharge planners to generate and facilitate new referrals. Patients and families will look to you also to help evaluate their needs for rehab and long term care services.
What it takes
- Bachelor’s degree in business, marketing or nursing required; or
- Two years’ experience in a long term care setting preferred
- Strong knowledge of the full continuum of care and eligibility requirements for home health, hospice, skilled nursing, and assisted living required
- Must possess a valid driver’s license and acceptable driving record
