Adult Case Manager
Woodfords is growing and has an opening for a Full-time Adult Case Manager within the Community Case Management Adult Program. The Community Case Management Adult Program is designed to provide support to adults with special needs and their families. Case managers help link individuals to resources and services in their community.
Responsibilities:
- Meets program productivity expectations as outlined by Program Supervisor.
- Meets regularly with consumer in a variety of settings to assess consumer functioning, progress, quality of life, health and safety issues, and to address concerns in this area.
- Seeks collateral contact with family and community providers to assess consumer needs and progress.
- Identifies and reports any suspected or observed abuse and/or neglect of an adult.
- Assists adults with maintaining a stable living arrangement and supporting their abilities to function in the community, vocationally, socially, financially and emotionally.
- Provides triage to consumers with emergency situations and provides information to consumers on emergency preparedness.
- Possesses working knowledge and understanding of MaineCare Sections 13:12, Section 21 and Section 29.
- Shares with consumer's agency and state information regarding Consumer Rights and Grievance Procedures.
- Attends regularly scheduled meetings with DHHS.
- Attends all required trainings by DHHS and/or as determined by Program Supervisor.
- Maintains up to date action notes, V-6 assessments, BMS-99 and other requested EIS requirements as determined by DHHS.
- Completes all Person Centered Plan responsibilities including and not limited to service planning and related documentation. The case manager will coordinate and monitor the plan and services throughout the year and make necessary changes if needed.
- The case manager will initiate and maintain positive and cooperative relationships with other community supports and provider systems.
Qualifications:
- Must possess a Bachelor's Degree from an accredited four-year institution of higher learning, with a specialization in psychology, behavior health, social work, special education, counseling, rehabilitation, nursing, or a closely related field.
- Must possess at least one-year experience in the field of developmental disabilities.
- Must possess a valid State of Maine driver's license and have use of a vehicle and proof of insurance.
- Must possess good organization, communication, decision-making, and human relation skills.
- Must possess the ability to deal with the stress of the position in an appropriate manner.
- Must possess the mental, physical and emotional health to carry out the position of Case Manager of the Adult Case Management Program.
- Computer Skills and attention to detail when writing.
