We are seeking Physicians who desire the opportunity and experience of delivering quality driven and compassionate care to our most vulnerable populations in the comfort of their home. 

The Nurse Practitioner delivers primary health care and patient care management in  residential and assisted living facilities. Physician manages reasonable daily case load (6-8 patients) via face-to-face visits. 

Qualifications

• Experience in Geriatrics or complex patients preferred.
• Active/unrestricted license and DEA
• Board Certified 
• EMR experience

*Excellent base salary + annual bonus*
