Job Summary and Qualifications

The advanced clinical pharmacist shall practice the profession of pharmacy with the utmost code of ethics demonstrating the organization’s mission and values. The clinical pharmacist is responsible for the safe, effective, evidence-based, and cost-effective selection and utilization of medications for the patients in which they serve. The clinical pharmacist uses advanced knowledge, skills, and professional judgment in all aspects of his or her work. The clinical pharmacist serves as a patient advocate when planning and implementing medication therapy within an interdisciplinary rounding team, collaborating with other healthcare professionals in a knowledgeable and professional manner. Adherence to all regulatory and medication safety guidelines are at the forefront of all actions and decisions made within the care process. The clinical pharmacist will work directly with the certified pharmacy technician to review home medication lists to provide a streamlined medication reconciliation transition into the hospital. Cost-effective utilization of medications will be reviewed and drafted as part of the inpatient admission medication list and discharge medication list by the pharmacist. As needed, the pharmacist will provide recommendations and/or literature to prescribers supporting the use of formulary agent, as well as, education to clinicians and patients regarding medication management.

What qualifications you will need:

• B.S. or PharmD from an ACPE-accredited School of Pharmacy
• A current and valid license as a registered pharmacist in the state of California
• National Provider Identifier (NPI)
• Preferred Qualifications: Completion of post-graduate hospital PGY1/pharmacy general practice residency AND at least one year of clinical pharmacy experience within area of focus, Completion of a post-graduate hospital PGY2/pharmacy specialty residency, 3-5 years of hospital clinical pharmacy experience with supervisory experience
