Are you passionate about making a difference in patients' lives? St. Luke's is seeking a dedicated Ambulatory Care Pharmacist to join our vibrant team at the Hailey Family Medicine Clinic. What You Can Expect
- Empower Patients: Work under a broad collaborative practice agreement to provide expert chronic disease state medication management.
- Fast-Paced Environment: Manage a dynamic clinic schedule with 8-12 patient appointments each day, ensuring efficient and effective care.
- Support & Educate: Be the go-to resource for providers and patients, answering drug information questions, troubleshooting pharmacy issues, and educating on medication use.
- Collaborate for Success: Coordinate with a multidisciplinary team, including physicians, advanced practice practitioners, behavioral health specialists, and care managers to deliver holistic care.
- Teamwork: Work alongside clinical pharmacy technicians to enhance patient outcomes.
- Mentorship Opportunities: Share your knowledge and experience by precepting pharmacy students and residents, shaping the future of pharmacy practice!

Qualifications
- Degree from an accredited pharmacy program
- Valid Idaho Pharmacist License
Preferred Qualifications
- Previous experience with the clinical management of primary care based chronic diseases
- Completion of PGY2 Ambulatory Care OR completion of a PGY1 pharmacy residency program with ambulatory care-focus OR equivalent ambulatory care experience
- Current Board Certification in Pharmacotherapy (BCPS), Ambulatory Care Pharmacy (BCACP), or Geriatric Pharmacy (BCGP)
