Position Overview:
-You will be working in the Sutter Sync program, which is a comprehensive care management program designed to virtually manage patients with chronic conditions such as hypertension or diabetes using remote patient monitoring tools and other various pieces of technology. Pharmacists will manage a panel of patients in collaboration with a team of Health Coaches and can adjust medications or offer personalized lifestyle advice to patients from the comfort of their home.
-We expect a heavy onsite presence at our Innovation Center in San Francisco for the first few weeks or months of the project. After we are stable, there will be an opportunity to work remotely, though additional onsite time may be required as we add new conditions.
-This role will be the first hire of many, with an expectation that you will help shape the final pharmacist workflows and assist with creating educational and onboarding materials/workflows for additional pharmacist hires prior to the program launch in March 2025.
Oversees and delivers pharmaceutical care through the provision of patient-centered clinical service, medication information, education, medication preparation and distribution that ensures safe, effective, and cost efficient medication therapy. Exercises sound judgment, and developed clinical skills to provide input to and implement the patient's plan of care based on the diagnosis in a timely manner. Gains confidence and cooperation from the patient, their family/support group, and other healthcare providers through competent patient assessment, attentive monitoring and care, and effective communication. Adheres to all local/state/federal regulations, codes, policies and procedures to ensure privacy and safety while delivering optimal patient care. May also be responsible for performing specific procedures and/or teaching duties. Uses professional judgment and clinical expertise in the daily solving of complex problems.

Doctorate: from an accredited college of pharmacy
CERTIFICATION & LICENSURE:
PHARMD-Licensed Pharmacist
TYPICA EXPERIENCE:
3 years recent relevant experience .

Pay Range is $100.70 to $120.84 / hour
_The salary range for this role may vary above or below the posted range as determined by location. This range has not been adjusted for any specific geographic differential applicable by area where the position may be filled. Compensation takes into account several factors including but not limited to a candidate’s experience, education, skills, licensure and certifications, department equity, training and organizational needs. Base pay is just one piece of the total rewards program offered by Sutter Health. Eligible roles also qualify for a comprehensive benefits package._ __
