• Must have active state license
• BC or BE required
• Weekdays, Days
• ACLS and BLS needed
• Up to 30 patients on clinic days (Mon & Wed), 25 - 28 surgeries on surgery days (Tues & Thurs)
• Epidural steroid injections, SI joint injections, medial branch blocks, radiofrequency ablations etc
• Credentialing needed
• DEA needed
