3. Weekdays, Days, Call, 2 weeks on / 2 weeks off, Weekend call
4. 2500 cases per year
5. General, urology, ophthalmology, ENT, orthopedics, OB, GYN, thoracic
6. Credentialing needed
7. DEA needed
