Job Summary and Qualifications

The Intake Specialist is to deliver exceptional, patient-focused care by providing level of care assessments to our patients at time of admission. Our clinicians work in an interdisciplinary team to best determine the appropriate level of care for patients. In this role you will work overnight to support our referral teams in the community and any walk in needs we may receive. Our Intake Clinicians are vital to the success of our programs and serving our community mental health needs.

What you will do in this Role:

• Conducts one-on-one assessment of patient at time of admission
• Works with Physician team to determine best level of care
• Assesses environment and patient’s safety status and reports/responds to actual and potential safety issues or changes in the patient's condition.
• Identifies risk factors, screening criteria, and interventions for patients with special precautions (e.g., suicide, homicide, falls, elopement).
• Completes assessments and all other documentation promptly and in compliance with requirements, as appropriate to scope.
• Conducts post-discharge follow-up as indicated.

What qualifications you will need:

• Basic Cardiac Life Support must be obtained within 30 days of employment start date
• Nonviolent Crisis Intervention must be obtained within 30 days of employment start date
• Masters Degree
• Licensed Clinical Social Worker (LCSW), or Licensed Professional Counselor (LPC), or Licensed Marriage and Family Counselor (LMFC), or Licensed Master Social Worker (LMSW), or Lic Pro Counselor Intern
