Assistant Director of Nursing 2 (ADON2) - Greene Meadows Nursing and Rehabilitation Center

 *Must be an RN 
* Previous ADON experience & proven track record required. 

Job Duties & Responsibilities:

• ADON2: (cover 3-11 or 11-7 callouts)

  1. Clinical management and oversight of units
  2. Progress Note review
  3. Admissions f/u:
  1. Assessment completion
  2. Review of future MD appointments
  3. Dx confirmation,
  4. Med rec for admissions
  5. Care plan completion
  4. GDR
  5. Care conference attendance
  6. Unit manager support- clinically
  7. Utilization review
  8. Wounds
  9. Rehospitalizations
  10. Ensure safe discharge for homeward-bound residents which may incorporate patient/ family teaching (meds and treatments).
  11. assist with recruitment/ retention efforts, and on-boarding.
  12. Promote and improve the nursing care of residents
  13. Supervise Nursing staff for policy compliance, behavior, and appropriate resident interactions, and initiate the disciplinary process
  14. Prepare QAPI material for the nursing department
  15. Relieve DON for time off
  16. Narcotic management
  17. Ability to prepare DOH-requested information and send secure files
  18. Ensure follow-up consult appointments are scheduled, completed, and reviewed with the MD
  19. Coordinate contract providers and facilitate barriers to care
  20. Positive support with the Interdisciplinary Team (IDT)
