Description
Integrity Pharmacy (a division of Clarest Health) specializes in Medication Management for patients with multiple disease states who are taking multiple medications. Our comprehensive pharmacy solution includes an in-home patient assessment; medication reconciliation; customized adherence packaging; monthly refill management and home delivery. Our Medication Management program helps eliminate duplicate medications through medication reconciliation and helps reduce hospitalizations and readmissions that result from medication errors. Our medication management program helps to ensure medication challenges are minimized and the highest outcomes for each patient we serve are achieved.
Job Summary
Staff Management:
- Ensure appropriate hiring and onboarding of new staff
- Ensure staff is appropriately licensed, registered and/or certified
- Ensure staff perform functions permitted by regulatory scope of practice
- Ensure staff are trained, including participating in, or providing coaching and training
- Ensure compliance training is completed as scheduled for new hires (upon start) and existing staff annually
- Ensure annual reviews are completed per schedule
- Ensure accurate management and documentation of PTO/Attendance records and requests
- Identify and communicate with HR business partner any staff performance/behavior concerns and development opportunities
- Provide secondary review of performance and annual reviews for staff reporting to supervisors in assigned departments.
- Ensure leads and supervisors are appropriately trained, engaged and meeting expectations
- Ensure coaching, counseling and discipline are appropriate and consistent with company standards and expectations including appropriate documentation
- Manage staff performance including coaching/counseling, new hire feedback and reviews, disciplinary actions, and annual staff performance reviews
- Ensure all staffing and scheduling is appropriate for workloads and meets any regulatory requirements
Regulatory Compliance
- Ensure compliance with ALL applicable Regulatory Agency rules and regulations including but not limited to:
- DEA
- Board of Pharmacy (including relevant PDMP processes/reporting)
- Department of Health
- Department of Aging
- FDA
- CMS
- Ensure notification to regulatory agencies (i.e.. Board of Pharmacy, DEA, Department of Health, etc.) of certain information as required by regulation is completed timely and accurately
- Ensure compliance with Practice Standards including, but not limited to:
- USP 797
- USP 795
- USP 800
- Ensure compliance with Company Policies and Procedures
- Ensure every task performed at the site is done within the appropriate scope of practice of the staff member and in compliance with any applicable regulations
- The ability to adhere to Clarest’s Code of Conduct, follow Clarest Compliance policies and procedures, and report any suspected violations of any federal or state laws to either their direct supervisor, Human Resources or the Compliance Officer
- Ensure compliance with ALL applicable Regulatory Agency rules and regulations including but not limited to:
Productivity Management
- Ensure staff meet or exceed established standards for performance including productivity, accuracy and quality
- Ensure all cutoff times and completion schedules and release times are met
- Identify opportunities to improve operations including, but not limited to, staff training, cost reduction, productivity improvement, increased capacity, improved quality, enhanced service, and best practices workflows and receive any necessary approval before implementation
- Ensure consistent, accurate and timely onboarding of new customers
Quality Improvement
- Ensure Quality concerns are appropriately documented and reviewed
- Ensure Quality concerns, when warranted, are escalated
Departmental Responsibilities – including, but not limited to:
- Responsible for the understanding and working knowledge of the applicable pharmacy computer systems, including, but not limited to Framework, ECM, DocuTrack, and CIM as it pertains to order entry, dispensing, Pharmacist Verification (PV1/PV2) functions, problem solving, quality and clinical review to ensure accurate, timely prescription/order processing and dispensing
- When needed, assist with pharmacist responsibilities
- Ensure all staff involved in pharmacy activities are properly trained and follow established policies and procedures and standards of practice
- Ensure sterile and non-sterile compounding processes are conducted in compliance with all applicable rules, regulations and standards, including USP 797, USP 795 and USP 800
- Ensure sterile and non-sterile compounding processes are conducted in compliance with Company Policies and Procedures
- Maintain all required certifications and provide oversight of cleaning/environmental management
- Ensure only technicians and pharmacists with appropriate certifications participate in compounding activities
- Ensure appropriate acquisition and storage of medications
- Ensure appropriate packaging, labeling and transport of medications
- Ensure proper management and documentation of drug acquisition and disposition including drug recalls and management of outdated/unusable medications
- Ensure compliance with pedigree documentation requirements
- Ensure proper execution of NDC flips
- Ensure technician activities are appropriately supervised in accordance with State requirements and scope of practice
- Ensure technicians follow established Company Policies and Procedures
- Where permitted by Board of Pharmacy regulations, ensure technician trainee programs comply with all applicable requirements and ensure appropriate documentation is maintained
- Ensure all required CDS documentation is maintained
- Ensure appropriate security with limited access to controlled substances is maintained
- Escalate any CDS fraudulent prescription, inventory discrepancy or potential theft or loss of CDS
- Ensure complete, timely and accurate information is provided to advise physicians, nurses, caregivers, residents, etc. regarding all aspects of pharmacy services including, but not limited to: drug therapies, Insurance/Third Party payable alternatives, alternative options during drug shortages, assist with medication administration guidance (i.e.. resident with PEG tube, resident with swallowing difficulty)
- Problem solve and resolve client issues or refer to other departments as appropriate
- Provide assistance in the development, deployment and management of department and customer supported initiatives
- Assist Billing, Central Rejects and Clinical Service Center with questions when appropriate
- Assist with Third Party Audits
- Ensure OSS process is managed appropriately, including, but not limited to, appropriateness of medications and quantities included in OSS, management of outdates, security of OSS in facility and during transport, performance of timely audits and maintenance of appropriate documentation and compliance with CDS OSS discrepancy process
- Ensure all packaging/pre-packaging is done consistent with applicable rules, regulations and standards of practice
- Ensure appropriate and accurate labeling of packaged/prepackaged medications including NDC, manufacturer identification, lot numbers and expiration dates (BUD’s)
- Ensure accurate documentation is maintained
- Ensure returns (refused or recalled medications) are processed in compliance with all applicable regulatory rules and consistent with Company Policies & Procedures
- Ensure records clearly and accurately indicate product disposition
- Ensure all required documentation is maintained
- Ensure all records requests are reviewed by corporate compliance and appropriately managed and reviewed
