Fairview Health Services supports continued learning on the part of its providers in order to maintain and enhance competency, licensure and board certification. Providers deemed eligible can receive up to $3,000 annually and up to 5 days of paid time off for CME’s.
Responsibilities/Job Description:In 2015, the Agency for Healthcare Research and Quality named MHealth Fairview Clinics as one of eight “exemplar” integrated behavioral health primary care models across the country. Integrated care is a result of an engaged primary care team and Behavioral Health Clinician providing patient-centered care addressing mental health and substance abuse conditions, health behaviors, stressors, social determinants of health, and ineffective patterns of health care utilization. At MHealth Fairview, integrated care is a “team sport.”
We are currently hiring for the Behavioral Health Clinician (BHC) position, a key role within a multi-disciplinary team in the Integrated Behavioral Health model. We are looking for behavioral providers with strong diagnostic and clinical skills, team engagement, and patient care skills who enjoy innovation, variability in your work day, and being a part of an exceptional team.
A Behavioral Health Clinician (BHC) is a fully integrated Mental Health Professional and core member of the primary care team that works under the direction of the primary care provider(s). The role of the BHC is to support the primary care team in the early identification and treatment of behavioral health conditions within the defined clinic population. The primary functions of the BHC are to:
- Complete diagnostic evaluations
- Provide consultation to the team regarding diagnostic impressions, recommendations, and strategies to engage patients in health and behavior change
- Support the team with the day-to-day management of complex patients.
- Provide brief evidence-based direct care to patients.
- Engage in population health activities to support risk stratified populations
Masters degree in social work, marriage and family therapy, or psychology with emphasis on clinical practice. MN state license, eligible to be licensed in the State of Minnesota and eligibility to become a credentialed provider with insurance entities. Experience in providing diagnostic assessments and direct evidence-based clinical services to children and adults. Experience with Electronic Health Record systems preferred.
Required:
- Masters or Doctoral Degree in Social Work, Marriage and Family Therapy or Psychology
- Licensed Social Worker (LICSW), Licensed Marriage and Family Therapist (LMFT), Licensed Professional Clinical Counselor (LPCC) or Doctoral Level Licensed Psychologist (LP)
- Licensed in WI or willingness to obtain prior to start
- 2 years post licensure
Preferred:
- 5 years post LICSW/LMFT/LPCC/LP licensure
- Previous experience completing diagnostic assessments, and providing individual, couples, family counseling for children and adults
- Healthcare experience
- Knowledge of health conditions and common co-morbidities
- Knowledge of the continuum of behavioral health care services (i.e. within surrounding communities)
- Experience with Epic
