About the Job
DUTIES/RESPONSIBILITIES-
- Implements a screening and triage process to assist families in determining the necessary services or assessments.
- Completes all data entry for pre-admission including identifying information, reason for referral, relevant history, and payor type information.
- Identifies emergent or complex cases and seeks clinical review, as needed, prior to finalizing the referral.
- Answers general questions and makes referrals for families/professionals seeking a range of Behavioral Health services. May support general administrative functions within the department.
- Gathering referral and pre-admission information from referring agencies, professionals, clients and other customers.
- Support the department process of "warm-hands" and appointment reminder process.
- Maintains working relationship with front office, operations, and clinical staff to assure efficiency in the day to day functioning of the resource and scheduling service.
- Employs a data-base to assess the demand/waiting lists for services and the ability of current service capacities to meet the community's needs.
- Ensures completion of Social Determinants of Health Questionnaire with the patient, referring them to internal and external resources as applicable
- Participates in outreach activities as designated by Chief Behavioral Health Officer.
- Participates in departmental and organizational quality improvement projects.
- Assists with other duties as assigned by Chief Behavioral Health Officer and/or Chief Operations Officer
- Maintains confidentiality of all clinical and administrative information consistent with NSCHC policy.

- Must have basic computer skills
- Demonstrates strong ability for problem solving in crisis or conflict situations
- Local travel between clinic sites
- Commitment to NSCHC faith-based mission to provide care to all who request it, without judgment
QUALIFICATIONS (REQUIRED FOR EMPLOYMENT) -
- Bachelor's Degree in psychology, social work or a related area is required
- Experience working with an Electronic Medical Record system preferred
- Minimum of 1 year of direct patient care experience required
- Act 33 with renewal
- Act 34 with renewal
- Act 73 FBI Clearance with renewal
- Annual Influenza vaccine
