Behavioral Health Navigator - HMSA

Job Title: Behavioral Health Screening Specialist

Responsibilities:

• Conduct telephonic screenings of BH members using PHQ-9, GAD, and other CM-DM diagnostic tools.
• Assess and determine additional care support needs for members.
• Communicate effectively with HMSA members, physicians, case managers, and disease management teams.
• Coordinate with colleagues to ensure comprehensive care delivery.
• Document findings and follow up on care plans as needed.

Qualifications:

• Relevant degree in psychology, social work, or related field.
• Experience in behavioral health assessment and care coordination.
• Strong communication and interpersonal skills.
• Familiarity with PHQ-9 and GAD tools preferred.
