Bilingual Case Management Coordinator - The Center

Reports to: Community Casework Manager 

Classification: Full Time | Non-Exempt 

Salary & Benefits: $55,000 - $60,000. Schedule: Generally Monday through Friday. Work days and hours may shift depending on scheduling needs; typical schedule will be 10:00 a.m. to 6:00 p.m.; flexibility required.

Summary: The Bilingual Case Management Coordinator helps address constituent needs by providing constituents with intakes, case management, referrals, and connecting constituents with programming across The Center. The Coordinator is part of the Center Support team, which includes Case Management Services through the Office of Victim Services, Patient Navigation, social support groups, Economic Initiatives and HIV and Sexual Health Services, and will work collaboratively with each team. This role will also work across other programmatic areas, including Mental Health Services and Substance Use Treatment Services.

Essential Duties and Responsibilities:

• Provide ongoing case management for constituents including writing case notes, identification of needs/needs assessments, ongoing meetings, safety planning, referrals and connections, advocacy with medical providers, educational providers, immigration, law enforcement, life-skills, and other city and state agencies.
• Confirm receipt of services from both external and internal referral appointments made for community members.
• Support, create, and implement a variety of engagement strategies to empower immigrant, undocumented, and asylum-seeking LGB, Trans and Gender Expansive communities. 
• Support the scheduling of intakes and conduct intakes, including maintaining paper records, collecting demographic information and details for grant reporting and  inputting them into data systems and ensuring all intakes have received necessary follow-up steps and assignments.
• Work and coordinate with front desk and administrative staff to ensure constituents move smoothly through the intake process and into program assignment.
• Provide support for all Center Support teams.
• Assist in linking constituents to both internal and external resources including referrals and follow-up support and maintaining contacts.
• Participate in all Center Support meetings and other relevant Center staff and grant funding related meetings, along with regular meetings with members of the Center Support team for supervisory and program planning purposes.
• Other duties as assigned.

 Position Requirements:

• Two (2) to four (4) years minimum experience and success in the provision of case management, mental health, community, and social services support.
• Bilingual or multilingual required. Language preferences: Spanish, Bengali, Russian, French, and/or French-Creole. 
• Excellent administrative, organization, and written/verbal communication skills.
• Strong interpersonal skills and ability to work with a wide range of work styles and cultural backgrounds.
• Detail-oriented and ability to handle multiple tasks at once.
• Previous trauma-informed support experience in working with individuals who are survivors of crimes and their families, including survivors of intimate partner violence, hate crimes and violent crimes.
• Knowledge of behavioral health resources in the greater New York City area and treatment regulations and programs.
• Commitment to inclusive, multicultural programming; prior experience working with diverse communities and an understanding of intersectionality
• An understanding of the concepts of institutional and structural bias, racism, and gender identity inequities, and their impacts on the communities served at The Center 
• Knowledge and understanding of the issues, needs, and interests of LGBT communities of color, youth, adults, families and community, including knowledge of the spectrum of gender identity and transgender issues, substance use, mental health, and homelessness
• A strong commitment to the mission of The Lesbian, Gay, Bisexual and Transgender Community Center.
