Responsibilities:
Initiate contact with pregnant/parenting women whose screens are received from Central Intake, other MSF Friends Programs, and community referrals to engage them in activities
Complete intakes with Program participants
Determine level of basic needs and connect families to services
Provide pregnant/parenting women and families with case management, screenings, and parent education services at the Center
Conduct or co-facilitate a variety of parenting education, stress management, and family health workshops/groups with pregnant and parenting participants
Enter Program data related to case management activities into the HRSA and ETO data bases as required and complete all required follow-up surveys
Distributes Pantry food bags and other items available as needed
Welcomes visitors to the Parent Child Center (families, staff, guests, deliveries, etc)
Assists families in completing applications for benefits for which they may be eligible i.e. SNAP
Introduces families to the option to complete a Family Wellness Plan
Communicates daily with Family Centered Services Coordinator/Case Manager for assignments/activities
Completes Family Intakes as necessary and assigned by Case Manager/Coordinator
Answers phone calls at Program desk and relays messages/directs calls as needed
Qualifications:
Bilingual (Spanish/English) Required
Case management/social working experience
Quick-learner
Passionate about helping the community
Pay Details: $21.00 per hour
