Description: Provides and directs Physical Therapy services to in home patients, including assessment, treatment, program planning and implementation, related documentation, and communication. Functions under physicians' orders.
Responsibilities Include:
· Provides Physical Therapy services to in home patients and initiates referrals when appropriate and in accordance with department procedures.
· Conducts assessments of in home patients at regular intervals to determine the need or continuation for intervention/treatment.
· Evaluates in home patients to obtain data necessary for treatment planning and implementation.
· Conducts specialized evaluations as indicated.
· Adheres to established confidentiality standards.
· Implements and conducts treatment as outlined in treatment plan.
· Follows management direction.
· Performs other duties as assigned.
Education and Experience:
• Bachelor of Science in Physical Therapy from an accredited program.
• Successful completion of National Certification Examination for Registered Physical Therapist.
• Current state licensure or license eligible.
• 0-3 years of long term care experience.
