Job description for a BRIDGES Counselor
We are seeking an extraordinary, kind, and compassionate leader who can acts as an advocate, supporter, educator, and caregiver to individuals with disabilities. What is a BC I & BC II position working with clients consist of along with the job responsibilities?
Implementing and following the person-centered, positive practice behavior support plan for individuals to maintain or improve areas of independent living such as budgeting, maintaining services, meal prep/planning, employment, etc.
Meet or exceed direct client time target of 90% with no more than 10% indirect time (driving, training, etc.).
Demonstrate a working knowledge of consumer(s) history, skills, vulnerabilities, support plans and emergency procedures.
Provide support and supervision as defined in each support plan, using therapeutic interventions, while ensuring consumer safety.
Report improper or suspicious behavior, misconduct of co-workers, abuse, or neglect of residents internally to the DOS, or externally to the Minnesota Adult Abuse Reporting Center within 24 hours of becoming aware of the incident.
Encourage local community setting/resources by remaining within a 10 miles radius of the clients' home.
Regularly travel in the community, transport clients, and assure safe vehicle operation including all passengers wearing a seatbelt, respecting speed limits, following agency policy, maintaining current car insurance, and always possessing a valid driver's license.
Ability to develop and maintain professionally appropriate and respectful therapeutic relationships with each client and the client's family; ability to maintain positive working relations with staff and other related persons.
Maintain a clean, professional, and pleasant appearance for effective role modeling to the residents.
Compensations: $18.00 to $19.00 per hour based on experience
Sleep Overnights $10.85 between the hours of 12:00 pm to 6:00 am
Requirements: For BC I & BC II
You must be at least 18 years of age and be able to read, write, and speak English in a fluent manner.
A valid Driver's License, clean MVR & auto insurance (You will be using your personal vehicle to transport clients)
The ability to pass a MN DHS background check
A working mobile device that can receive & send phone calls and capability of downloading application for documentation
The ability to make independent decisions
Reliability and flexibility are necessary for success in this position

Additional Requirements: For BC II
A Bachelor's Degree in Psychology, Social Work, or a related field preferred
OR an Associate's Degree in a Social Services Discipline
OR two years (4,174 hours) of supervised experience working with individuals who exhibit challenging behaviors as well as co-occurring mental disorders or neurocognitive disorder.
