BSW/LSW Aftercare & Outreach Coordinator
Reports to: Site Director
Major Functions: Performs duties related to the Aftercare Program, Community Referral Program, and outreach responsibilities for nursery and community services for Providence House.
Providence House is a drug-free workplace, and all applicants are required to complete a criminal background screening and a 5-panel drug screening prior to starting employment.
Additionally, all employees and volunteers are mandated reporters.
Aftercare
- Provide Aftercare program support to include making reminder calls/texts to families, managing roster, and leading group educational sessions three to four times a month using the Nurturing Parenting, Conscious Discipline, and S.E.L.F. curricula.
- Complete case management services to all families attending group parent education. Document all contacts appropriately and timely.
- Facilitate monthly pickups and deliveries of the Aftercare Care Packages for qualified families without transportation
- Complete outreach and recruitment for monthly speaker series to enhance Aftercare programming
- Plan and execute quarterly special events for the Aftercare community.
- Document all contacts with parents or guardians, referring agencies, and community partners using agency forms and spreadsheets.
- Complete monthly calls for all RTIS assigned families to support family engagement post-discharge. Partner with the assigned social worker for support.
- Provide support to the Program Team assigned for nursery families Community Referral Program
- Manage relationships with Community Referral Program partners, leading onboarding orientations as needed and recruiting additional partners to support the distribution of basic needs to community members.
- Track and report data related to families served in the Community Referral Program
- Complete outreach meetings with partners to ensure utilization of Providence House as referral
Outreach and Education
- Complete outreach and tabling events for Providence House partners to expand the community’s knowledge and utilization of
Providence House services
- Attend monthly collaborative meetings to support agency awareness and resource partnerships in the geographic areas the nurseries serve
- Complete speaking engagements on behalf of the agency to referral partners as requested
- Complete quarterly education and basic need distribution events in collaboration with partners to address needs in the communities served by Providence House
Administrative Tasks
- Document all contacts with clients and partnering agencies (phone-based, in person meetings, team meetings, referrals, etc.).
- Attend program team meetings, management meetings, and outreach meetings as assigned
- Assists in leading internal training sessions that addresses staff understanding of the psychosocial needs of clients served.
- Provide Quality Assurance on all client files and report deficiencies to Director of Programming and Clinical Operation and Compliance and Quality Manager
- Know and understand rules pertaining to childcare as outlined in 5101:2-5 and 5101:2-9, CARF accredited areas, and OMHAS and Medicaid standards and requirements.
- Undertake other assignments and projects as requested by the Site Director or other staff acting as a Project Manager.
- Strive to be culturally competent and supportive of diversity in both service to clients and through professional interactions with donors, partners, and guests
- Assist with other duties as assigned.
- Assist with the flow and sorting of inventory for Aftercare and Community Referral and distribution events in partnership with the Facilities Team.
- Reports allegations of abuse or neglect to the Cuyahoga County Division of Children and Family Services
- Be a part of the on-call rotation with the Program Team to address the after hours needs of the nurseries
Qualifications:
- Bachelor’s degree or higher preferred with BSW and eligibility for LSW; must have LSW within 90-day probationary period
- Must complete trauma training through National Institute for Trauma and Loss in Children through the e-certification program within six months of hire
- One year experience in an administrative support position or related position
- Computer proficiency in MS office suites and database software programs
- Excellent communication skills
- Strong multi-tasking, time management and organizational skills
- Excellent proofreading and data entry skills
- Meet 40 lbs. lift requirement
- Ability to think creatively and work confidentially
- Flexibility in adjusting to agency needs
- Positive team-oriented attitude
- Must be available for occasional weekend or evening work/modified schedule based on agency activities
- Must be able to travel in the Northeast Ohio area and have reliable transportation
