Cardiac Nurse RN Case Manager - Weill/Cornell - Days

Primary Responsibilities:
Coordinate comprehensive care for patients with complex cardiac conditions, including heart failure and left ventricular assist devices (LVADs).
Collaborate closely with an interdisciplinary team to assess patient needs, develop individualized care plans, and facilitate safe transitions from hospital to home, rehabilitation, or skilled nursing facilities.
Lead discharge planning and organize services such as home health nursing, physical therapy, and home infusion support, with a focus on patients discharged on intravenous inotropes or requiring long-term LVAD management.
Discharge Planning and Patient Throughput:
Identify and mitigate discharge barriers by coordinating necessary resources and solutions to support safe and timely discharges.
Promote optimal patient throughput by expediting transitions and minimizing hospital length of stay.
Actively work to reduce hospital readmissions through comprehensive discharge planning and follow-up care.
Patient and Caregiver Education:
Educate patients and caregivers on medication protocols, device management, and post-discharge care, enhancing patient and family understanding of long-term cardiac care needs.
Arrange for durable medical equipment and coordinate follow-up appointments to ensure ongoing patient progress and support health management at home.
This role is critical in supporting chronically ill cardiac patients to effectively manage their health at home, fostering improved long-term outcomes and quality of life.
This is a day shift, Monday - Friday position.
Preferred Criteria
2 years of case management/care coordination experience in an inpatient, HMO, insurance, or other outpatient setting
Cardiac Experience
1 year of leadership experience in a healthcare setting
Master's Degree in healthcare related field
Required Criteria
Bachelors degree in Nursing (BSN)
Current NYS Registered Nurse (RN) licensure
3 to 5 years of health care experience in an acute care setting
Superior communication, demonstrated problem solving skills, strong business acumen, and interpersonal skills are critical

Salary Range:
$100,000-$140,000
