- Compensation: - Base salary $96,000 - $137,000/yr- Annual bonuses up to $20,000

- Additional Info: Opportunity to join a dedicated and diverse team working together to provide quality care and patient-centered care coordination.Responsibilities:- Provides direct care and education to Health Center patients and ensures continuity and quality of care. - The provider will have a solid understanding of patient needs and will work collaboratively with a multidisciplinary team to co-manage primary care patients.Qualifications:- Must be eligible for or already hold an active Massachusetts license as a Physician Assistant.- Must maintain a current Massachusetts Controlled Substance Registration (MCSR) and DEA registration.- Must apply for and be granted privileges at affiliated hospital(s).- Understanding of ambulatory family practice health care program administration and clinical operations preferred.- Experience with Patient Centered Medical Home model and concepts preferred.- Fluency in Spanish and/or Portuguese a plus.
